- No Form D has been filed under Beacon Health’s own name.An EDGAR search for the company turns up only two unrelated filers: Beacon Health Holdings LLC, a Massachusetts entity with filings from 2009 to 2013 that show no evident connection to this company, and the SLRTE I LLC SPV above.
